Terms of Service

SIGNATURE
By signing Lake Restorations' services agreement(s), the client acknowledges and agrees to the terms and conditions outlined herein. The
signature indicates the client's acceptance of the services to be provided, the associated costs, and any other
stipulations mentioned in the signed document. The client also allows photographs and/or videos of the treatment area
to be taken prior to, during, or after each treatment. Lake Restoration, Inc. may use this content for marketing and/or
educational purposes. There is no stated or implied guarantee of results, and payment is due upon completion of
treatments regardless of results. Lake Restoration, Inc. may share information with relevant parties regarding the
client’s service status. This information will not be sold to outside sources.
TERMS
By signing a services agreement, client hereby makes application for a permit to destroy or control aquatic vegetation or aquatic nuisance
algae as described in the document. I understand that the control of aquatic nuisances, including destruction of aquatic plants
and algae, is subject to rules of the Commissioner of Natural Resources. I understand that an Aquatic Plant
Management Specialist may wish to inspect the proposed treatment area before, during, and/or after work is completed
and that by making this application, I give permission to the specialist to enter my property to make such inspection at
reasonable times. I understand that an annual report may be required on results achieved. Completion of this form and
processing of the accompanying application fee does not constitute obtaining a permit.
Invoices will be sent out prior to each treatment with net 30 terms. By accepting the services of Lake Restoration, Inc.
for aquatic plant control, the client waives the right to hold Lake Restoration, Inc. responsible for any injuries, damages,
or losses that may occur during or after the pesticide application. The client acknowledges that the use of pesticides is
inherently risky and that Lake Restoration, Inc. is not liable for any damages resulting from the use of pesticides. The
client agrees to indemnify and hold harmless Lake Restoration, Inc. from any claims, damages, or losses arising from
the use of pesticides. If cancelling early when using the early sign-up discount or the prepay discount, the client will be
billed the discounted amount for each treatment completed.
SERVICE GUARANTEE
The service guarantee ensures Lake Restoration, Inc. will get adequate control of aquatic plants within the treatment
site. If the client’s treatment site does not have 50% or more control of aquatic plants 14 days after the treatment has
occurred, please call to discuss with one of our lake experts. The service guarantee is limited to one time use. To receive
the service guarantee, the client will be required to provide photographs of the entire area that was treated, as well as
the weeds present. With all the programs offered, there is no residual control. Re-growth or replacement by other weeds
or organisms may occur. The following weeds are excluded from the service guarantee because they can be resilient to
herbicides: Vallisneria, Chara, and clasping leaf pondweed.
